Self Assessment Tool for COVID-19

Are you experiencing any of the following:

  • Discomfort or tightness in the chest
  • Unusual shortness of breath
  • Severe abdominal pain
  • Prolonged and persistent headache or dizziness
  • An injury that may require stitches or involve a broken bone
  • A child who has prolonged diarrhea or vomiting; or
  • A baby under six months of age with a fever of 38C (100.4F) or higher

Have you returned to Prince Edward Island from outside the province within the past 14 days?

Have you been in close contact with someone with a confirmed case of COVID-19?

Close contact means:

  • Providing care for someone who tested positive for COVID-19
  • Having close physical contact with someone who tested positive for COVID-19
  • Living with someone who tested positive for COVID-19 (who isn’t able to properly and consistently socially distance)
  • Having close lengthy contact with someone who tested positive for COVID-19
  • Having contact with bodily fluids of someone who tested positive for COVID-19

Public Health proactively follows-up with all identified close contacts of someone who tests positive for COVID-19.

Have you been in close contact with someone who has returned to Prince Edward Island from outside the province in the past 14 days and who has a fever, a new cough or cough that's getting worse?

Close contact with someone means:

  • Providing care
  • Having close physical contact
  • Living with someone (who isn’t able to properly and consistently socially distance)
  • Having close lengthy contact
  • Having contact with bodily fluids

Public Health proactively follows-up with all identified close contacts of someone who tests positive for COVID-19.

Do you have a new onset fever or worsening cough, sore throat, rhinitis (runny nose, sneezing, nasal congestion post-nasal drip) or marked fatigue?

Please call 9-1-1 or go to your nearest emergency department.

You should call your family physician or nurse practitioner. If you do not have a family physician or nurse practitioner or are unable to reach them, call 8-1-1.

You must also self-isolate for 14 days and continue to monitor your health.

If you travelled recently, this means 14 days from the day you returned to Prince Edward Island.

If you had close contact with someone with confirmed COVID-19 or someone with symptoms who has recently returned to Prince Edward Island, this means 14 days from the day you had contact.

If you travelled recently, YOU MUST SELF ISOLATE FOR 14 DAYS. This means 14 days from the day you returned to Prince Edward Island.

If you had close contact with someone with confirmed COVID-19 or someone with symptoms who has recently returned to Prince Edward Island, YOU MUST SELF ISOLATE FOR 14 DAYS. This means 14 days from the day you had contact.

If you have NOT recently travelled, had close contact with someone with a confirmed COVID-19 or someone with symptoms who recently returned to Prince Edward Island, you are not required to self-isolate but must continue to practice social distancing. Stay home as much as possible. If you do go out, keep 2 meters or 6 feet apart from others.

All Islanders are reminded to monitor their health closely.

If anything changes, you can use this tool to re-assess.
